Use this tool to complete a questionnaire and then submit a movement screen all without holding a live session! This tool walks you through the process step-by-step. Once you have completed the questionnaire and familiarized yourself with the instructional movement screen videos, find a spot to record yourself performing the screen guided by the last step. Upon completion of the screen and you have a video (it should be less than 10 minutes), you need to submit the video on the CONTACT page of the website. As soon as I receive your video, I will review it. If I have any questions, I will reach back out to you. Once I have everything I need, I will assess you, create a report, and send you the detailed findings in your Crescendo Athlete Profile Report. Then you can use that information to either have me design you a program or do so on your own.
